You must call the Warmline at 1-800-439-4079 for pre-approval and guidance for initiating this process. Once pre-approved, you will be required to complete a Patient Syphilis Record Search Request Form and fax it back to the Warmline along with a coversheet that includes your facility or institution’s letterhead.
